To request a grant or funds from the Foundation for your health and education initiatives, complete and submit the corresponding form below:

This form is for chapters and/or non-profit organizations with a valid 501(c)3 status. These organizations must submit this form to request funds to be used for community service projects they are working with currently. The use of the Tax ID numbers for discounts or tax write-offs may also be requested with this form.
SHE Funds Request Follow-Up Form
This form is used to filter money from mentoring and/or scholarship programs through the foundation for chapters. The purpose for filtering funds through the foundation is for chapters to allow their contributors to use the 501(c)3 non-profit status to write off taxes.
SHE Funds Replacement Form
Once contributions from the foundation have been made, chapters or non-profits must submit a final report within 5 business days of the event. Funds donated to chapters and/or non-profits must only be used for donations to community services in education, health and/or scholarship.
